Role Purpose
The Quality Outreach Specialist will play a critical role in ensuring success within the Accountable Care Organization (ACO), Medicare Advantage and commercial contracts with a focus on Healthcare Effectiveness Data and Information Set (HEDIS) quality measures. This service-oriented position is knowledgeable in HEDIS quality measures and familiar with approaches for reviewing electronic medical records to identify and submit documentation to address quality measure gap closure. This position requires strong attention to detail, excellent communication skills and a passion for quality measure performance.
Responsibilities
Utilizes knowledge of quality measure specifications to aid in the audit and review of patient charts for quality measure information.
Conducts outreach to patients to address open quality care gaps; provides education and assists with determining the discrepancies between recommended best practices and the actual care provided.
Uploads supplemental data to individual payor portals towards meeting contractual quality metric requirements and improving performance in quality incentive programs.
Assists patients with scheduling appointments and placing orders for the care they need to remain healthy.
Assists with quality performance improvement plans and efforts; executes health campaigns, participates in meetings regarding quality; generates ideas to improve quality performance.
