SSM Health DePaul Hospital in Bridgeton, MO, and SSM Health St. Joseph West Hospital in Lake St. Louis are recruiting for an experienced Nurse Practitioner or Physician Assistant for our Complex Care Clinic. This is a PRN/Fill-In role that offers a competitive rate based on experience, malpractice coverage, but no other benefits.
Job Highlights:
- Outpatient only covering a minimum of two shifts per month at SSM Health St. Joseph West Hospital in Lake St. Louis and covering provider PTO and sick days as available at both SSM Health St. Joseph West Hospital in Lake St. Louis and SSM Health DePaul Hospital in Bridgeton.
- Complete pre-surgery evaluations and determine optimization needs based on algorithms and clinical pathways
- We focus on optimizing medical conditions and often coordinate care with patients' PCP or specialiststhere is limited ordering of testing and medications with this portion of the role
- The transitional care is a post-discharge clinic focused on preventing hospital readmissions. We evaluate patients within the first week post-discharge, assess status, and treat/refer as needed
- This role requires strong evaluation and assessment skills
- Limited diagnostic workups and prescriptions are involved in this setting
- Strong department onboarding and mentorship
- Fantastic team and department culture and collaboration
Requirements:
- Master's degree in Nurse Practitioner or Physician Assistant
- 2+ years' experience working as an NP or PA in Internal Medicine/Adult Primary Care
- Missouri registered nurse license and advanced practice nurse licensure or certified physician assistant
