Summary:
As an integral member of the pharmacy team and under the supervision of a pharmacist, is responsible for working with newly admitted patients to obtain and document an accurate medication list to help avoid medication duplications, omissions, and errors. The information obtained will be entered into the electronic medication database system or updated in the database if information already exists. This information will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. This position is responsible for providing technical support in all aspects of the delivery of safe, effective, patient focused, and cost-efficient drug therapy. Knowledge of pharmaceutical terminology, metrics systems, and computer systems is necessary. In addition, customer service skills and the ability to effectively work and communicate with patients, staff, and outside providers is part of the requirements for the role. The medication history specialist may provide patient care, treatment, and services that are within the scope of their license, certification, registration, and/or assessed competencies, but will be prohibited from and providing services that require professional judgement regarding the preparation or distribution of drugs. Practice will be in accordance with laws and regulations.
Requirements:
High school diploma
Previous experience as a Pharmacy Technician 1 or successful completion of LVN training (required).
1-3 years in a pharmacy retail setting (preferred).
1-3 years as a Health Care Professional with knowledge of medications, medication terminology, and computer systems
Current registration with the Texas Board of Pharmacy (Certified Pharmacy Technician) or current licensure with the Texas Board of Nursing (Licensed Vocational Nurse)
