Atlantic Health System is seeking a Dietitian, Atlantic Visiting Atlantic Visitng Nurse at Morristown Medical Center, hours would be 8a-4p weekdays. This role would encompass being a part of an interdisciplinary team as a visiting RD providing care to a primarily geriatric population in their homes. This includes but is not limited to assessment, management, and education regarding dietary requirements and restrictions related to the patients diagnosis and physicians orders. A valid NJ RD License required along with a NJ DL. Previous hospital or homecare experience preferred.

• Performs as a member of the health care team and provides nutrition care plans based on approved criteria for clinic patients.

• Identifies patients at high nutritional risk based on criteria outlined by the clinic.

• Develops and implements the nutrition care plan for all assigned patients.
• Continuously evaluates and monitors the effectiveness and outcomes of medical nutritional therapy intervention. Keeps data according to goals.
• Provides nutrition education to patient and family members on appropriate food selections and eating behaviors to accomplish patient goals.

Qualifications/Requirements:

• Must be able to read, write and speak English

Minimum Education:

• BS in Nutrition/Dietetics or equivalent required

License/Certifications:

• Must be a Registered Dietitian

Relevant Experience:

• Experience documenting in an EMR preferred
