Overview
Atlantic Health System is seeking a Dietitian, Atlantic Visiting Atlantic Visitng Nurse at Morristown Medical Center, hours would be 8a-4p weekdays. This role would encompass being a part of an interdisciplinary team as a visiting RD providing care to a primarily geriatric population in their homes. This includes but is not limited to assessment, management, and education regarding dietary requirements and restrictions related to the patients diagnosis and physicians orders. A valid NJ RD License required along with a NJ DL. Previous hospital or homecare experience preferred.
Responsibilities
Performs as a member of the health care team and provides nutrition care plans based on approved criteria for clinic patients.
Identifies patients at high nutritional risk based on criteria outlined by the clinic.
- Develops and implements the nutrition care plan for all assigned patients.
- Continuously evaluates and monitors the effectiveness and outcomes of medical nutritional therapy intervention. Keeps data according to goals.
- Provides nutrition education to patient and family members on appropriate food selections and eating behaviors to accomplish patient goals.
Qualifications
Qualifications/Requirements:
- Must be able to read, write and speak English
Minimum Education:
- BS in Nutrition/Dietetics or equivalent required
License/Certifications:
- Must be a Registered Dietitian
Relevant Experience:
- Experience documenting in an EMR preferred
