The incumbent is responsible for policy development, quality resources, and specialty functions unique to the Primary Care Mental Health Integration (PCMHI) to optimize patient outcomes. He/ She will monitor, develop, and evaluate the activities of the PCMHI. This position is also announced as a Psychologist, Licensed Mental Health Counselor and Social Worker- see Duties section for more information. 8. Coordinates with discipline supervisors of matrixed staff assigned to these programs to ensure staff are working in accordance with PCMHI. 9. Meet with Primary Care PACT teams and extended team members at assigned clinics on a regular basis to ensure timely patient appointments, treatment planning, services offered, and solicit regular feedback on additional service offerings needed. 10. Facilitate bimonthly staff meetings with all PCMHI staff to review any clinic issues, patient consultation, appropriateness of consults to PCMHI, staff burnout, and programmatic discussion. 11. Education(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. Grade Determinations Clinical Pharmacy Specialist (Facility Program Manager) GS-660-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Programmatic oversight includes policy development, quality resources, and specialty functions unique to the PCMHI to optimize patient outcomes. This includes setting the agenda for the programmatic goals and delegating responsibility to the staff for achievement of those goals. Goals of the program will be dictated by local, VISN, or National VA goals and directives. The progress of programs initiated and performed by the PCMHI staff will be reported to the Associate Chief of Staff for Mental Health. 1. Responsibilities include oversight of the PCMHI administrative and programmatic resources for the interdisciplinary team that are specialized and complex and significantly impact the delivery of health care service to Veterans in the STVHCS as well as the Primary Care PACT teams. Responsibilities also include development and implementation of the PCMHI program across all Primary Care Clinics in South Texas, inclusive of policies and procedures, individual and group therapies that maintain fidelity of the PCMHI model, supervision, and monitoring of outcomes using a data-driven logarithmic assessment tool for effective Care Management, called Behavioral Health Lab. Monitoring access to timely service delivery of therapies and treatments by use of same-day access in Primary Care clinics.\n2. PCMHI Program Manager manages all day-to-day work of the program and its staff and programmatic changes, alterations to clinic staff schedules, reassignment of staff to other clinics, review clinical demands and requests and ensure adequate coverage of clinic during holidays and other seasonal demands.\n3. Manages the work of the program and its staff, ensuring that work is assigned in a systematic way and meets facility and VA guidelines/standards.\n4. Monitors program outcomes using data-driven quality assurance processes.\n5. Coordinates programming with the various mental health program managers and teams, as well as with Primary Care and Pharmacy providers in PACT teams. Assists in meeting the necessary standards for PCMHI such as appointment wait times, coordination of group room space, office space throughout the main hospital and CBOC's for the PCMHI staff, monitoring open encounters and unfinished Clinical Reminders by PCMHI\nstaff. Also ensures team member compliance with performance measures.\n6. Review policies and procedures with PCMHI staff to ensure compliance with performance measures related to access and clinic utilization rates.\n7. Supervisory duties for assigned staff to include mid-year and annual performance evaluations, OPPE or FPPE for new and existing clinical staff as applicable, review productivity. Responsible for planning and approving leave such that clinical functions are maintained within clinical areas managed.
