KEYRESPONSIBILITIES
Obtainsand documents a healthhistory;performs and documentscomplete,system-focused,orsymptom-specificphysicalexamination, assessment and planofcare.
Incollaboration,provideshealthcareservicesforprimary, acute and complex careofpatients with kidney disease and hypertension and managespatient'soverallcare,identifyingexpectedoutcomesfordiagnoses.
Orders,performs,interpretsand collectsdatausing appropriate assessment techniques, relevantsupportingdiagnosticinformation and diagnosticprocedureswhereindicated.
Formsdifferential diagnoses and treats acute and chronicconditions, as appropriate.
Prescribes and revisestherapeutic interventions and medical regimensbothpharmacologic (including immunosuppressive therapy) andnon-pharmacologicnecessarytoachieve expectedoutcomes.
Provides patient education and counseling in the areas of health promotion, maintenance, and disease prevention
Utilizesevidence-based,approved practiceprotocolsinplanning and implementingcare
Initiates appropriate referrals and consultations
Providesspecialtyspecificconsultationservicesuponrequest and withinspecialtyscopeofpractice
Facilitatesthepatient'stransitionbetween and withinhealthcaresettings,such as admitting, transferring, and dischargingpatients.
Provides care coordination and non-patient facing care such as communication with local community providers and review of medical records and results.
Participates in team meetings and conferences to enhance an interdisciplinary approach to health care delivery
Participates and leads chronic kidney disease and treatment options sessions for the CKD patients.
Participates in post-hospital discharge follow up of patients with kidney disease
Department Summary: The Division of Nephrology and Hypertension at Vanderbilt University Medical Center has a long and notable history of studying the pathophysiology of and developing potential therapies for the prevention and treatment of kidney disease.
