JOB SUMMARY: Demonstrate the ability to communicate effectively both written and verbally with homeless clients and system staff and supervisors. To provide eligibility and enrollment assistance for the Medicaid, Medicare, CHIP, and Federal Marketplace health insurance programs to uninsured patients of the health center and homeless community residents. Conduct in-reach, outreach and education activities to existing health center patients and non-health center patients about affordable insurance options and help them enroll, and perform data collection.
MINIMUM QUALIFICATIONS
Education: High School Diploma or GED
Licenses & Certifications: Basic Life Support from a hospital- based American Heart Association (AHA) approved program, CDL Certificate;Other certificates as required by unit/service preferred.
Work Experience: Two (2) yrs. experience in Public Health, Outreach, Social Work or related field.
Language Skills: Bilingual - Spanish preferred but not required

Other Requirements: Must be available by phone or pager after hours. Travel to outreach and worksites in the community. Working knowledge of the Texas Medicaid Program, CHIP, and the public assistance benefit application preferred.
