Chilton Medical Center, part of Atlantic Health System is seeking a Part Time Social Worker ( MSW ) for their Case Management dept.

Part Time, Day time shift with weekends and holidays on rotation.

Essential Functions:

• Provide social work coverage to facilitate discharges.
• Meet with patients and families to discuss and support the plan of care and engage them in discharge planning.
• Provide resources, referrals and support as needed on a case by case basis.
• Coordinator transitions of care to extended care facilities, inpatients behavioral health and chemical dependency facilities, hospice and palliative care programs, LTAC and Rehab.
• Attend daily huddles and interdisciplinary rounds and work closely with the healthcare team to implement the plans of care.
• Obtain all necessary third party authorizations, communicate with community resources and area facilities.
• Daily use of phone, fax and electronica documentation in EPIC.

Education:

• MSW with current NJ State Social Work license. LSW or LCSW.

Qualification / Requirements

• Communications skills
• Problem solving,
• Organization, ability to work simultaneously on more than one case.
• Time management a must.
• Phone, fax and computer skills, willingness to work with a team.
• Compassionate service delivery.
• Ability to work with all age groups.
• Hospital experience and documentation in EPIC experience preferred.
