General Description of the Job Class
As a member of the pharmacy leadership team, this manager is responsible for working collaboratively with the managers and Associate Chief Pharmacy Officers to coordinate optimal patient transitions of care for medication therapy at Duke University Hospital.
Duties and Responsibilities of this Level
- Works with pharmacy leadership, other health care providers, and relevant policy-making committees to establish policy and implement initiatives that improve patient outcomes and overall quality of patient care.
- Provides leadership and supervision for the responsible practice area(s) and staff including monitoring staffing needs, scheduling, and assuring appropriate staffing levels are maintained.
- Maintains relationships with nursing, medical staff, and program leadership to achieve division, pharmacy department, hospital, and health-system goals.
- Assists of the planning, design, implementation, and evaluation of pharmacy department policies and procedures, and ensures staff compliance with the same.
- Provides leadership and leads staff development projects (design, implementation, measuring impact).
- Supports, coordinates, develops, and actively participates in the medication history, and transitions of care programs.
- Coordinates the completion, tracking, and documentation of quality standards related to transitions of care activities and programs.
- Monitors and evaluates current and new pharmacy program initiatives related to medication use and transitions of care.
- Participates in recruiting and selection of staff.
- Maintains familiarity with all departmental programs.
- Leads and/or participates in committee, workgroup or team meetings, including the DUHS Medication Reconciliation Task Force.
- Participates with student and resident education.
- Provides educational lectures (Duke, State/Local, and National).
- Takes responsibility for personal and professional growth.
- Maintains manager staffing responsibilities and assists with completion of medication history and discharge processes.
- Provides on call assistance for area of responsibility.
- Maintains skills and practice as a clinical pharmacy practitioner

Minimum Qualifications
Education
- Doctor of Pharmacy Degree OR BS Pharmacy Degree
Experience
Two years of pharmacy practice experience; Managerial/supervisor experience preferred
Degrees, Licensures, Certifications
- Licensed Pharmacist or eligible to practice pharmacy in North Carolina
- Certification in Basic Life Support (BLS).
