We are seeking an energetic, motivated Social Worker that wants professional autonomy and understand the importance of care coordination. The Case Manager is a vital member of the interdisciplinary team that is responsible to create, implement, and monitor treatment plans to ensure safe, timely, and effective transitions throughout the care continuum and discharge planning.
PRIMARY FUNCTION:
To assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources/ services through the provision of a full spectrum of discharge planning and social work services. The successful candidate will work 8a-4:30p Monday-Friday.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Masters degree in Social Work from an accredited graduate school of Social Work.
DE LMSW requires within 6 months of hire. LCSW preferred.
Two years of social work experience in an acute care setting preferred.
SPECIAL REQUIREMENTS:
CCM required within 18 months of eligibility
