This role will work closely with the Director and the Clinical Managers. The intent is that the HHC agency (hospital based) has a connection to the care management team for UNMH. This role will assist with difficult home health placement of complex patients and serve to educate about UNMH Home Care services.In home observations will also be a part of this role to support the field staff in practice. With many changes in documentation this RN will assist in review and education. The intent of the position is that this is a very robust role to play in the support of various facets of the agency.
Supports the goals of the Director of Home Health which encompasses those of UNMH. Supports the transition of patient from hospital to home. Responsible for the patient referral analysis and evaluation process, development of improved work methods, and for providing staff training. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
Accountability:
COMPLIANCE - Ensure conditions of participation are followed in tandem with home health and hospice management
PATIENT CARE - Responsible for the analysis and evaluation of patient referrals to ensure all standards are adhered to for a safe discharge
PATIENT CARE - Participate in patient care conferences, case manager assignments, and attends appropriate rounds on the units
PATIENT CARE - Responsible for the development of improved work methods and procedures to achieve and maintain improved outcomes and clinical goals of the department
EVALUATIONS - Responsible for field evaluations for clinical care in the home
EDUCATION - Provide and coordinate clinical learning experience for staff and students
CUSTOMER SERVICE - Ensure quality customer service through training, monitoring and coaching; resolve customer service issues and problems
DOCUMENTATION - Documents in PowerChart communication of patient acceptance or rejection for referral to home care, when option discussed with patient
PATIENT SAFETY - Ensure that the patient is discharged safely to the home when referred to UNMH Home Health
PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
Education Requirements:
Associate's Degree in Nursing
Experience Requirements:
2 years directly related experience
Licensure/Certification Requirements:
Licensed Registered Nurse (RN) in State of New Mexico or as allowed by reciprocal agreement by State of New Mexico
CPR Certification for Healthcare/BLS Providers or for Professional Rescuers or must obtain within 30 calendar days of date of position
Education Requirements - Preferred:
Bachelor's Degree of Science in Nursing
Experience Requirements - Preferred:
Home Health or Hospice Experience
Physical Demands Requirements:
Heavy Work: Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects or people. Physical Demand requirements are in excess of those for Medium Work.
Testing Requirements:
Tuberculosis testing is completed upon hire and additionally as required

Sig Hazard: Chemicals, bio hazardous materials requiring extensive safety precautions and may require protective equipment
