Clinical Coordinator Pharmacy Full Time Rotating Position Summary
Organizes and coordinates the activities and flow of work for the clinical pharmacy.
Provides leadership and serves as a role model and resource person for clinical and staff pharmacists. Leads, analyzes, and appraises clinical pharmacy services provided to patients, physicians, and ancillary departments. Coordinates implementation of clinical initiatives to develop a cohesive environment with consistent direction and goals.
Overseas safe and appropriate pharmaceutical care for patients by developing and utilizing evidence-based and best-practice protocols. Develops and implements policies and procedures. Promotes and supports safe medication management.
Shares the responsibility for formulary review, drug use evaluations, therapeutic drug protocols, drug information, and development and maintenance of clinical pharmacy services.
Initiates, analyzes, and reviews monthly and annual reports to determine compliance with clinical initiatives and value propositions. Implements, maintains, and reports adverse drug reaction reporting.
Develops and implements policies, procedures, and protocols with management and medical staff.
Acts as consultant by coordinating education for pharmacy, nursing, and medical staff regarding medication therapy and drug information through programs, in-services, newsletters, or other means.
Must be able to perform staff and clinical pharmacist duties as necessary for patient care. Performs essential duties of pharmacy manager or director in his/her absence. Supervises and directs pharmacy personnel, provides clinical competency input for pharmacist's annual evaluation, and assists management in interviewing/hiring/training new personnel.
Monitors clinical interventions, adverse drug events, non-formulary and target drug use.
THE CLINICAL COORDINATOR PHARMACY FULL TIME ROTATING CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.
Required: Valid current Arizona Registered Pharmacist License. Basic Life Support (BLS-CPR). Advanced Cardiac Life Suport (ACLS), within six (6) months of hire.
Preferred: Pediatric Advanced Life Support (PALS)
Required: Graduation from an accredited school of College of Pharmacy with a PharmD. Obtains certification in pharmacotherapy or the subject of his/her specialty through the Board of Pharmacotherapy Specialties (BPS) within 36 months of assuming the position.
Preferred: Specialized education/training residency
Required: Minimum three (3) years clinical pharmacy in acute care setting or a one (1) year PGY1 residency. Must be able to work variable shifts including nights, weekends, and holidays.
