JOB SUMMARY: To build and maintain positive relationships with healthcare providers in the Vanderbilt Health Affiliated Network by engaging with providers to join VHAN, to actively participate in VHAN and to improve their patient care quality and cost metrics. Facilitate and smooth implementation of IT tools across network providers. Disseminate performance reports, protocols, VHAN provider communications and other clinical initiative information. Gain accurate, timely information from participating providers that informs network planning and operations, clinical service opportunities, overlap areas, and meta data on referral patterns and admissions. Provide feedback to hospitals & service lines expressed by providers in the VHAN Network. Interface with participating providers and facilities as appropriate.
Understand VHAN content:
Gain a thorough understanding of the business model and be prepared to articulate the value proposition for each party
Gain an understanding of the Infrastructure components and the value of each component to the Practices
Gain an understanding of clinical measures, protocols, performance reports, etc. Deliver performance reports to Practices with context and convey next steps
Prepare written communications:
Presentations that accurately and persuasively convey key messages
Letters that document decisions, memorialize points of agreement and accurately describe facts and circumstances
Newsletter articles that inform and persuade. All written communication shall be professional. It shall uphold and build the Network's reputation
Be confident, energetic, accurate and persuasive
Conduct meetings to achieve purposes that are stated and understood by meeting participants
Conclude meetings by summarizing next steps; what, who, when
Listen to what is said so that it can be conveyed accurately and so that follow-up is responsive
Ask clarifying questions to gain understanding
Summarize all meetings by restating and clarifying next steps; who, what, when. Complete and communicate results within agreed upon timeframes.
Build trust by meeting and exceeding Providers' expectations
Respond promptly to issues or concerns expressed by community providers.
Report issues and service recover efforts to VHAN Leadership for analysis, identification of trends and intervention, as appropriate.
