We are seeking a full-time, experienced therapist to join our talented team of professionals. This position is in our Adult Partial/Day Hospitalization Program which provides quality, evidenced-based clinical interventions to individuals requiring more intensive mental health care than what a weekly counseling or psychiatric appointment can provide. It also serves as a step down from inpatient psychiatric stays.

Responsibilities include: conducting individual and group sessions as well as family meetings, developing individualized treatment plans, coordinating post-hospital care, and documenting all patient contacts in the electronic medical record. Acts as primary therapist, providing a range of therapies, crisis intervention and behavior management to achieve improvement of treatment goals and discharge dispositions. Provides case management and coordination of treatment services for children, adolescents, and their families to facilitate access to needed treatment. This may include coordinating with referring professionals, treatment team, QI, UR, and others to ensure appropriate care. Conducts intake sessions to assess patient and family needs. Completes documentation of assessments, treatment plans, therapy sessions, case manager discharge summaries and formal written reports as needed. Participates in staff meetings and professional committees as assigned. Provides staff or community in-services when assigned. Participates in activities integral to the maintenance and continued development of program. Provides assessment, planning, treatment, intervention, and evaluation to patients with addictions.
